MAKING AN APPOINTMENT
When you initially call Gastrointestinal Associates you will be asked for some initial information including:
- Name and address
- Date of birth
- Name of primary
- Type of insurance
plus insurance numbers
- Social Security
- Problem bringing you
to our practice
You will be given an appointment with one of our doctors. You will be sent forms (or, you can download them here) to complete and bring with you on the day of your appointment. You will be asked to get any medical records that may help with your care. Please bring these with you to your first appointment.
When you call, your demographic information will be reviewed for changes. If it has been a year or more since your last visit you will be sent forms to complete (or, you can download them here). If you have not been seen in a year or more of if you have a new problem we will want any appropriate medical records brought with you to your visit. If you have had any changes of significance since your last visit to our practice please let our phone operator know.
Open Access Colonoscopy
Gastrointestinal Associates has developed a program which allows healthy individuals to schedule screening colonoscopy without the need for an office visit before the procedure. If your physician has suggested that you have a colonoscopy you may qualify for this program. Of course, not all patients will be able to safely undergo colonoscopy without a more detailed evaluation of their health history and their risks for the procedure. If that is the case for you, we will help you schedule an office visit so that a physician can review your medical history, assess your current condition, and determine how to best meet your health needs.
