Gastrointestinal Associates has developed a program which allows healthy individuals between the ages of 45 and 75 to schedule screening colonoscopy without the need for an office visit before the procedure. If your physician has suggested that you have a screening colonoscopy you may qualify for this program.
Of course, not all patients will be able to safely undergo a colonoscopy without a more detailed evaluation of their health history and their risks for the procedure. If that is the case for you, we will help you schedule an office visit so that a physician can review your medical history, assess your current condition, and determine how to best meet your health needs.
To submit the Direct Access Colonoscopy questionnaire online, you may fill out the form below. Please note you should have the following information available:
- Your insurance information
- Details regarding any prior colonoscopies (dates, locations, rendering physician)
- Your complete medication list including prescriptions and over-the counter medicines (vitamins, supplements, etc.)
Your information will be reviewed by our staff and you will be contacted within 10 business days to schedule a colonoscopy if you qualify.
