Insurance Information

Gastrointestinal Associates, Inc. (GIA) participates with Medicare and most private insurance plans. We also participate with most, but not all, Medicare replacement plans. If you are considering us for your GI care, the best way to determine if we accept your plan is to call your insurance company and verify with them.


Please be sure that you understand the financial responsibility you may have, such as co-pays, co-insurance or deductibles. GIA is mandated to collect all co-pays, deductibles, or other fees which are your responsibility under your insurance plan. Your co-pay is due on the day of your visit. We accept cash, check, debit cards, and credit cards (Visa, Mastercard & Discover).


It is the responsibility of the patient to ensure that we participate with your plan. If you change your insurance coverage between being scheduled and the appointment date, you should contact our office to update your insurance.

What to Bring to Your Appointment

Please bring the following to your office visit or procedure appointment:

  • Driver’s license or other photo ID
  • Your health insurance card
  • Secondary insurance card, if applicable

Referrals

If your insurance requires a referral from your primary doctor, please call for that at least one week prior to your appointment so that your doctor’s office has time to process the referral.


Your doctor’s office may need our practice NPI number to process the referral. Our group NPI # is 1013436542.


You are personally responsible for any fees not covered by your insurance and you will be billed by our office.

Procedure Billing

If you have a procedure, typically there will be 3 separate charges: one each for the physician, the anesthesiologist, and the facility. If specimens are taken, there may also be a bill from the lab which processes them.


If you have questions regarding any bill, it is important that you call the phone number noted on the bill itself. The facility, lab and anesthesiologist are not part of GIA, and we will not be able to assist with these concerns.
